Print out the form, complete it and remember to sign it. The completed and signed form can be dropped off at any of ACA's three Centers during office hours or mailed to ACA. Completed forms collected by ACA must be turned in before the October 11, 2016 deadline so the new registered voters can vote in the upcoming November elections.
